This video demonstrates a surgical technique for Dacryocystorhinostomy using the endonasal approach. Surgeon: Dr. Patrick Yang, Oculoplastics Surgeon, Canada
Well, almost bloodless! Here are a few tricks that can be done to have a Bloodless DCR surgery: 1) A well-relaxed patient: A mild sedative on the day of surgery helps. 2) Blood pressure: Adequate control of blood pressure 3) Blood thinners: Stop all blood thinners 5 days prior, in consultation with Internist. 4) Good local anaesthesia (Xylocaine with adrenaline), even if you are performing it under GA 5) A slight head-up position during surgery helps prevent bleeding due to passive congestion. 6) Good nasal pack with Xylocaine and adrenaline; although I only use 3 Johnson buds. 7) Operating under surgical loupe magnification. This allows the visualization of fine blood vessels. 8) A good Monopolar and Bipolar cautery: Electrical, or Radiofrequency 9) Anatomy: There are specific locations where bleeding occurs: Orbicularis/Angular vessels/bony perforators/sac mucosa/nasal mucosa. Knowing these specific locations of bleeders will help you predict it and control it. 10) Blanching the nasal mucosa with Xylo+adrenaline injection just before the flap making (as shown in the video) helps relieve pain and allows bloodless flap. 11) Top up anaesthesia: Pain has to be controlled, to avoid the anxiety-induced rise in blood pressure. Use more local anaesthesia at specific points during surgery, or prefer GA (my personal choice). Thanks for watching. Dr Milind Naik, MD

Dacryocystorhinostomy is a surgery done to relieve watering eyes that occur as a result of blocked tear drainage. DCR surgery is the surgery of choice in nasolacrimal duct obstruction (NLDO). If done meticulously and with appropriate anesthesia techniques, a DCR surgery can be painless for the patient and bloodless for the surgeon. Usually this surgery is performed by oculoplasty surgeons . Oculoplastic surgeons are highly skilled specialist plastic surgeons of the eye who treat conditions such as droopy eyes, watering eyes, eyelid swellings, spasms of the eyelids and face, prominent bulging eyes, thyroid eye disease, scars, eyelid bags and dark circles around the eyes.

Lacrimal, bicanalicular, silicone, stent, intubation, DCR, Dacryocystorhinostomy, canalicular , sleeve, Bicanalicular silicone stent intubation is commonly performed during DCR although its benefit in the absence of canalicular obstruction remains unproven. This video demonstrates the technique of sleeve placement, adjustment and the ease of stent removal. Intubation related problems include: corneal irritation from a loose tube, canalicular damage and slitting from a tight tube and difficulties with tube removal, requiring nasal endoscopy. The use of a short silicone sleeve to stabilize the tube ends intra-nasally, in place of a knot or suture, prevents all three of the above problems by allowing precise adjustment and easy stent removal via the medial canthus.

Dacryocystorhinostomy (DCR) surgery is a procedure that aims to eliminate fluid and mucus retention within the lacrimal sac, and to increase tear drainage for relief of epiphora (water running down the face). A DCR procedure involves removal of bone adjacent to the nasolacrimal sac and incorporating the lacrimal sac with the lateral nasal mucosa in order to bypass the nasolacrimal duct obstruction. This allows tears to drain directly into the nasal cavity from the canaliculi via a new low-resistance pathway. External dacryocystorhinostomy (DCR) is the gold standard surgical technique for the treatment of nasolacrimal duct obstruction. Since its introduction by Gibbs in 1967, intubation with silicone tubes has been widely used in lacrimal duct surgery Following surgery, the patient is discharged home if stable and instructed to rest for one week, without heavy lifting, exercise, or strenuous activity that may induce bleeding. Hot drinks and food should be avoided for the first 12-24 hours postoperatively in order to decrease the risk of epistaxis caused by heat-induced nasal vasodilation. Ice/cold compresses are placed on the incision site for 48 hours while awake to minimize swelling and bruising. The patient’s head should remain elevated at all times at a 45 degree angle and the patient instructed to avoid nose blowing for one week to decrease the risk of hemorrhage. Skin sutures are removed one week postoperatively if nonabsorbable sutures were used, and the silicone tube is removed typically at 4-8 weeks after surgery.
